How much will it cost to replace my verison phone with insurance?

the first phone went through the washer, and was replaced for $40 with insurance, however, the replacement is now lost. im not usually this irresponsible with my phone, in fact, its never been broke or damaged or lost before. but, i need to know if verizon insurance will cover a lost phone, and if they do, will they allow me to use insurance 2x ?
